PASSPORT APPLICATION
| the ds-11 passport application can be downloaded from www.travel.state.gov or aquired through our office at the grand forks county office building at 151 south 4th street, grand forks, nd phone: 701-780-8251 | ||||
| fees | (1) | $75 - check to u.s. dept. state - for 16 yrs +. good for 10 yrs. | ||
| (2) | $60 - check to u.s. dept state - under 16 yrs. good for 5 yrs | |||
| (3) | $25 - processing fee payable by check or cash to grand forks county | |||
| forms | (1) | state certified copy of birth certificate | ||
| (2) | naturalization papers; or | |||
| (3) | most recent passport - this must be submitted - if lost or stolen must complete a lost or stolen form | |||
| photos | ||||
| two color passport photos - photos must have been taken within the last 6 months, they must be identical and they must be passport quality | ||||
| passport photos can be taken in the property & records department for a fee of $13, or at any of the local photo studios. check your local phone book for listings | ||||
| proof of identity | a current and valid drivers license or military id is required | |||
| effective feb 1st, 2008, as provided by case of federal regulations title 22, section 51.28 , passport applications for children under 16 years requires both parents signatures. see www.travel.state.gov for requirements | ||||
| all children must be present and must be accompanied by both parents if under 16 or one parent if 16 to 18 | ||||

| expedited passports | ||||
| passports can be expedited by including the extra $60
expedite fee with the regular fee. In some cases, (if needing the passport
within 10 days or less) proof of travel is required. This proof can be a copy of
the plane ticket, the travel schedule, etc. In such a case, the customer should bring in one Express Mail stamp purchased from the post office or the cash (no checks or credit cards) equivalent which is $ 17.50. This allows us to Express Mail the application to the passport agency. If the customer would like the passport returned by Express Mail, they include an amount of $ 14.85 with the payment to Passport Services (Dept. of State). | ||||
